Healthy ageing and perceptions of ageing

 This assignment can only be a maximum of 2000 words (not incl. references). Please refer to the OVERVIEW OF ASSIGNMENT & MARKING CRITERIA when completing this assignment. I have highlighted uploaded documents in the OVERVIEW OF ASSIGNMENT and the HEALTHY AGING WELL INITIAL ASSESSMENT that you will need to use to complete this assignment. I have uploaded the additional assessment I completed to be used in section 3. (Alcohol screen, Brief pain inventory, Epworth sleepiness scale, Geriatric depression scale & Home safety checklist). I have tried to cover all areas (risks and issues) of the patients health that need to be assessed from the HEALTHY AGING WELL INITIAL ASSESSMENT to complete section 3, if you can see any other risks or issues with the patients that I haven’t assessed can other areas that I have missed, can you let me know. This assignment is designed to demonstrate your ability to collect and process data, identify problems/issues for an older person and create a healthy ageing plan of care that supports the health and wellbeing of an older person. It’s about partnering with the older person to provide comprehensive care.
