Select a geographic area of your choice (e.g. Warren County or city of Cincinnati). Locate & research 10-15 services available to families of children with disabilities or people with disabilities in your community. The resources must be from a variety of service areas (such as psychological services, health/nutrition, vocational/employment, residential/community, social/recreational, summer interventions or recreation such as speech therapy, hippotherapy, camps, etc.). You may also consider selecting organizations that could be instrumental in successful transition planning for students as well and you can identify these as such (transition resources). Write a 1-page summary of each organization. The summary should have an overview paragraph giving a description of the organization followed by bullet points with key information. The summary must include: Name of the organization What they do or offer Who they serve, specifically, types and degrees of disabilities, if relevant, age of those served Contact information (here is where your research should turn up specific names or people to ask for if at all possible – we want to help families avoid answering machines and confusing menus if we can) Is there a waiting list? Is this a program that fills up way in advance, or requires special things in order to attend such as a physical exam etc.? Cost information (if applicable) including if they take Medicaid, insurance, scholarships, offer a sliding fee scale and if so what is the scale? How does a person register or sign up for this service? Is there someone to help a family who does not speak English or does not read? How would this organization benefit families/students with disabilities in your school? Any other relevant information you feel would help families when available. The required info in this guide should be MORE than you can find on the website or flyer. This assignment requires you to CALL the agency or resource to get more detailed info. I cannot stress this enough. You MUST go ABOVE AND BEYOND the basic info provided on a flyer and get the details. Do NOT copy & paste from the website unless it is directly pertinent or helpful. This project must have a cover/title page with all authors names and must be organized into categories by service or resource. All resources must come from one geographic area. You may choose the area. Make sure this is thorough, professional in appearance, well organized and well-written.
