The case study to use is: (You are a social worker with the Aboriginal Medical Service in Redfern and the doctors have referred a 32-year-old Aboriginal man with mild intellectual disability and physical disability (amputation as a result of mismanaged diabetes) as he needs support to obtain accessible housing and in-home support services. The man has lived with his mother, who is 55 years old and she has stated she can no longer care for him within the family home as she has recently started a new relationship and she wants time to herself. The man identifies with the Redfern area. Your role is to assist with housing and also coordinate support services for him.) The 500-word submission to Turnitin is written in the form of a case note. It is to take the form of a case summary. In the case note, it is necessary to detail the actions taken by you as a social worker and detail any actions you have allocated to the client and/or their family to take. This approach is the one  you follow for whatever scenario you have chosen to respond to. Remember, you cannot do absolutely everything for clients and they need to take some responsibility and ownership for the situation they find themselves in. When writing the case note remember personalised language is to be avoided, so no using ‘I’. Instead, when referring to yourself as a social worker, initially write your full name and in brackets insert your initials.eg. Grace Egwim (GE) and from that point only refer to yourself using your initials. Avoid using judgmental language such as ‘suffering’. Use person-first language and refer to the client as Ms. and choose a surname or Mr. and choose a surname, as often people with disability are treated in a child-like manner which is inappropriate unless they are children. Additionally, with the case note, it can be written in a combination of it being what is to be done in the future and what has already been done. E.g.‘I have done a housing application’ or ‘I will be referring them to a tenancy service in the future.’ Indicate timelines for your actions. E.g. ‘We will be meeting again in two weeks’ time to collate information for the housing application.’ The PowerPoint slides are meant to be an illustration of actions that you will take so you can discuss it with your colleagues in the form of a case conference or team meeting to discuss actions. In this, you can talk about different social work approaches to use, provide a summary, and again outline actions. The PowerPoint presentation is a way for you to practice communicating cases to your fellow colleagues. Do not make assumptions about the case studies provided. You can addinformation in because you need to make up a date of birth and detailsabout supports that they have. So applying for the National DisabilityInsurance Scheme (NDIS) might be an action you take for your clients. Remember the case studies I have provided are representative of a basic the outline you receive to facilitate an initial referral to whatever the organisation you are working with. This assessment still requires referencing in accordance with APA 6th Edition on the last slide of your PowerPoint presentation and also in the case note submitted to Turnitin. This means that you will be referencingorganisations websites. You can also reference any theoretical approaches you choose to use in responding to the clients need e.g. Grief and loss theory or strategies for working with families.
