A 52-year-old, who worked in the financial district of the city, complained of several weeks of intermittent headache, irritability, and inability to concentrate. He had previously enjoyed good health. Physical examination and routine laboratory tests were all normal, and within a few days he began to feel better. Three months later his symptoms recurred and persisted for several weeks. A lumbar puncture was performed and the cerebrospinal fluid (CSF) had a slightly increased protein level with five lymphocytes per cubic millimeter (normal is 0-3). India ink and Gram stains failed to reveal any microorganisms. A careful neurological examination was unrevealing and he again eventually felt better.
Two months later when he developed severe headaches, nausea, and a stiff neck. A repeat lumbar puncture showed an elevated opening pressure; the CDF had increased protein and decreased glucose levels, and the white blood cell (WBC) count was 300/mm^3, most of which were lymphocytes. Microscopic examination was negative, but a latex agglutination test for microbial antigen in CSF was positive,and culture of the CSF grew out a budding organism with a capsule. Subsequent investigation revealed that the window air conditioner in his office was heavily contaminated with pigeon droppings. He was treated with amphotericin B and flu cytosine, and he eventually recovered. His physician ordered tests of his immune function.
