a 4000 word report detailing how to tackle an identified health issue affecting a specific group or community. The health issue is CVD and the group is Black and Ethnic Minorities in London. What needs to change [determinant/behaviour] that are the major risk factors are Poor Dieting and Lack of Exercise. This exactly the same case in the sample essay I have attached however the health issue there is diabetes among Asians in East London. The interventions are Socio-Ecological Behaviour Model and Logical Model. The Report should include Ethical Considerations and set out action plan. This work is part of a presentation attached where there is a slide with a graph of all London regions clearly indicating the health inequalities to articulate the health issue. all red bars are regions where BME communities live. The report should borrow heavily from Department of Health’s ‘CVD Outcome Strategy’ and ‘NHS Health Checks in tackling CVD among BME in Glasgow’.
