Advanced Directives and Last Will & Testament
Create a word document where you will reflect upon your thoughts and feelings as you prepared this document for yourself – again DO NOT TURN IN THE ACTUAL LIVING WILL DOCUMENT TO ME …..just create a paper outlining your thoughts and feelings about completing this. Within your paper that you turn in to me – make sure to include your thoughts (how you would answer the questions – even if you cannot have children) on each of the following areas from the Advanced Directives link (Legacy Writer) in your assignment submission: 1. Terminal Condition 2. Permanently Unconscious 3. Tube Feeding 4. The Pregnancy Scenario 5. Pain 6. Euthanasia 7. Organ Donation Continue through each step. If you do not understand a question click on the area that is blue with a line under it and a box will pop up to define the term to you. Failure to comment on these areas will cost you considerable points. *** Please make sure to comment on each of these (a sentence or two) from your personal point of view. *** You may want to download and print the Living Will for your own personal records. If you decide to keep the Living Will document, remember to Initial, date and sign your document where requested. You need 2 witnesses to sign. Page 5 “Attestation of Witnesses” explains who CANNOT sign as a witness. Please read the guides and guidelines in the document box to fully understand the living will agents, signing and more information. Provide someone (or more than one person) with a copy of this document. Also, store a copy in a safe place (ex. fire proof safe or with an attorney). (2) In addition to the above it is also a good idea to have a Last Will and Testament (specify where your items and estate go to upon your death). The below link will help you understand how to create your own. It is not free but you are allowed to go through each step until a form of payment is requested. Therefore, go through each step to help complete your own which can be done by using Microsoft Word. Last Will and Testament (Links to an external site.) Fill in your information as requested and continue through each step. Use this to help complete your own word document. Print your personal document and have 2 or 3 witnesses to sign it. Please remember a witness is someone who does not receive anything (ex. a beneficiary), must be a legal adult and of sound mind (know and understand what they are signing). (3) After completing this second aspect of the assignment, write about how you felt completing it as well as the living will above, how it relates to the textbook content, and why it was important to this class. Generally 5 to 6 fully developed paragraphs (500 words minimum) is sufficient. This is the only item you will submit to me for grading (your thoughts and feelings about completing the Living Will as well as the Last Will and Testament and how this assignment relates to the material in the text and the course).
